Footwear and Orthoses

General MSK Principles

Overview

Footwear and foot orthoses are everyday tools in sport and exercise medicine (SEM), used to manage load, protect tissue and support rehabilitation across many lower-limb and musculoskeletal (MSK) problems. The right shoe or insole can offload a painful area, alter how force passes through the foot and leg, cushion impact or accommodate a deformity, while the wrong one can create or prolong an injury. This page covers how shoes and orthoses are built, how they are thought to work, which presentations they help, and how to choose between prefabricated and custom devices, drawing on the companion topics on gait analysis and running and movement biomechanics. The aim is practical, evidence-based prescribing rather than routine or automatic use.
