Motivation and adherence decide whether good advice becomes better outcomes. In sport and exercise medicine (SEM), most treatment is active: a rehabilitation programme after injury, a loading plan for tendinopathy, or a physical activity prescription for a long-term condition. None of it works if the person does not carry it out, and non-adherence to home exercise and physical activity is common, so supporting behaviour change is a core clinical skill rather than an afterthought. Adherence is the extent to which someone follows a plan they have agreed to, an active partnership better understood this way than as passive compliance. Knowing what drives motivation, recognising when adherence is slipping and why, and applying the strategies that help matter as much to recovery from a musculoskeletal injury as the exercise prescription itself.
