Overview

Hospital deconditioning is the loss of physical, and sometimes cognitive, function that results from inactivity and acute illness during a hospital stay, and minimising unnecessary inactivity is a central modifiable strategy for preventing it. The broad term hospital-associated deconditioning describes this decline, while the more specific term hospital-associated disability means a new loss of ability in everyday activities, which affects about 30 in every 100 acutely hospitalised older adults. Muscle strength, balance and fitness fall quickly with reduced mobility, and the effects reach beyond the muscles to mood, confidence and thinking. Alongside inactivity, acute illness, inflammation, poor nutrition, medication effects, delirium and the hospital environment all contribute. For the sport and exercise medicine (SEM) clinician, much of this harm is preventable: early, sensible activity during an admission helps protect strength, mobility and independence, and supports recovery.
